In partnership with local communities, CPCCO promises to focus on supporting the needs of OHP members within our communities, with special attention to children. Developing safe and healthy communities will help to support the emotional, social and health needs of our children and their families so that they can ultimately reach their highest potential.
By providing the resources we know are needed, we are helping to tackle issues such as the housing crisis, substance use disorder and other obstacles to a healthier community. All while building community resilience and working toward health equity.
You can find more about our community work, our most recent Annual Report and our plans for the future of the community on our About us web page.
For over a decade, Columbia Pacific CCO has worked closely with members, health providers and the community to improve overall health, give patients better care, and find ways to deliver that care more efficiently.
When Oregon Senate Bill 1580 passed in 2012, it provided a path for Oregon to create a new way to administer and coordinate health care for Oregon Health Plan (Medicaid) members. Coordinated care organizations (CCOs) were born as a result. CareOregon created Columbia Pacific CCO that same year to continue serving members in Columbia, Clatsop and Tillamook counties with this new coordinated care model.
Prior to 2012, Oregon Health Plan members might have had three separate plans for their physical, mental health and dental care. Providers for these types of care were separate – so they weren’t coordinating with each other.
With CCOs, physical, mental and dental health providers all work together in much more efficient ways to improve the care of every member. Instead of treating our members only when they get sick, we focus on preventing disease and helping people manage their chronic health issues (like asthma and diabetes) before they become more serious.
