Regional Health Needs Assessment and Regional Health Improvement Plan

 

Columbia Pacific CCO, our advisers and local partners engaged in a community conversation through 2018 and 2019. The conversation addressed the factors that create health and well-being for people in the three counties we serve.

Individuals shared their lived experiences of health and well-being. We also considered the need to support efforts to improve access to care in areas such as:

  • Public health departments
  • Hospitals
  • Clinics
  • Community safety net providers
  • Mental health organizations
  • The education system
The result was our five-year regional health improvement plan (RHIP). The RHIP helps improve public health. It does this by merging the values and needs expressed by the community with specific ways public health informs health care. Many value-based health innovation and improvement efforts will take place from 2020 to 2024, which will both reflect the opinions of those who gave their time and effort during our 2018-2019 planning process and take population health metrics into account.
